POLICY OF TRANSFERS TO OTHER LEVELS OF CARE

Valley View Retirement Community provides three primary levels of care. residential living cottages are provided in Valley View Village. Assisted living services are provided in Valley View Terrace. Around the clock nursing home care is provided in Valley View Haven.
As the health care needs of our residents change, they may require the services provided at another level of care. We believe that health care decisions requiring a resident to transfer to another level of care should be made after careful consultation with his or her physician, Valley View's administration, and the resident and his or her representative.
Residents who need admission to another level of care will receive Priority Access. In other words, our residents will be admitted to the levels of care they need with priority over, and before, individuals from outside the retirement community.
If the number of residents, requiring transfer to another level of care exceed the available accommodations, residents will be admitted on a first-come, first-served basis (determined by when our Social Services Department is informed of need of transfer to the Haven and when our Director of Assisted Living is informed of needs for transfers to the Terrace).
